As a health care consumer, you have the right to inform agency staff, without fear of reprisal, of any concerns you may have regarding care, services rendered, or treatment of your personal property. We encourage you to ask questions or share concerns with your primary care provider. If you have a concern that you feel has not been resolved with the staff, you may file a complaint directly with the agency by calling Certified Home Care at 517.364.8600.
If you feel that your problem has not been resolved by contacting the appropriate agency listed above, the Michigan Department of Community Health maintains a toll free hotline 800.882.6006 (8:30 am - 5 pm, Monday-Friday, with an answering machine available after hours, on weekends and holidays).
The purpose of the hotline is to receive complaints or questions about local home health agencies. You may also use this phone number to express any questions or concerns about the way your advanced directives have been implemented.
